Please download our New Patient Intake Form [PDF File], complete it as best as possible, and bring the completed paperwork, along with all of your medications and supplements, to your first visit. If you are unable to print the form, or if you prefer, we can mail the paperwork directly to your home.

Your first visit will be scheduled for up to approximately two [2] hours in length. This is to allow enough time for an in depth intake, analysis, and discussion of your current state of health. Subsequent visits will be scheduled for one-half [1/2] to one [1] hour. Phone consultations are available as well.

Located near Prospect Park in Brooklyn, the Park Slope Office is located on the first floor of a row house and is easily accessible via the F train [1 Block to the Seventh Avenue Station] or the R train [5 Blocks to the 9th Street Station]. MTA Buses such as the B67, B69, and B75 provide access as well. Parking is limited to on-street parking.
Winter office hours at this location are: Thursday and Friday: 10AM to 8PM.

The Manhattan Office is located in the Flatiron / Union Square Neighborhood just a few blocks from Union Square. Access via the subways at Union Square include the N, R, 4, 5, 6 trains and the F and V trains at 14th Street. MTA Buses such as the M2, M3, M5, M6, and M7 provide transportation. Parking lots are located on various adjacent blocks [18th, 19th, 20th] and vary in rates by the day of the week.
Winter office hours at this location are : Wednesdays: 8AM-12Noon and Saturdays: 9AM-6PM.

Fees and Payment Options

Any acupuncture therapies, if indicated, are included in the fees.
Supplements and Chinese Herbal Medicines are priced accordingly.

Payment is due at time of visit. Available options include Cash, Check, Visa, and MasterCard.
New York Natural Medicine does not accept insurance nor Medicare.

We offer discounts for Senior Citizens [aged 65+] and also for Students [up to 26yo].

We firmly believe that Health Care Access is a fundamental Human Right and will not deny care to any person because of an inability to pay the standard rates. We can work with you on a Sliding Scale basis to determine an appropriate fee for your individual situation.
